Post-completion changes that belong to no correction session.

A null correction_id means only that no session was open, NOT that no human made the change: enforce_response_lock also permits an authenticated admin or inspector to insert an is_amendment row on an amendment-mode inspection, which lands here with a real actor_user_id. Do not present these as system writes.

unattributed=true is required rather than defaulted, so this path stays free to mean "every revision on this inspection" later without silently changing what an existing caller gets.

include_start=true merges unattributed Start-section revisions (a service-role support fix, say) into the same sequence; without it the rows are body-only, exactly as before.
